A Name question for your forms
Ask for a name in separate boxes — prefix, first, middle, last and suffix — or in one box, and use the first name on its own in emails and Auto Flows.

Ask for a name the way people expect to give it.
What's new
- A Name question. Add it from the Basic group in the form builder. It asks for First name and Last name side by side, with First name required, until you change it.
- Choose the parts. Show or hide Prefix, First name, Middle name, Last name and Suffix, rename any of them, and choose which are required. Or switch to Single box for one box for the whole name.
- Arrange them. Put the parts on one row, one under another, or in two columns.
- Filled in for your visitors. Each box takes the name the visitor's browser has saved, so most people fill it in with a tap.
- Read back as a name. Responses, your CSV export and form emails show the whole name, such as "Dr Asha Rao", in one place.
- Greet people by first name. In an email, insert just the first name, or any other part, from Insert answer…. An Auto Flow started by the form can use the first name on its own too.
- Kept in step with your contacts. On a form that adds respondents to contacts, save the question to the contact's Name and it fills in their first and last name. A display name your team has set by hand is left as it is.
Also new
- Address questions can be set line by line. Show, hide, rename or require each line of an address on a form.
- Responses are named. The Responses list shows who answered from the form's Name question.
- Rules can read a name or an address. A rule such as "Name contains Rao" now works.
In the form builder, select Add question or content, then choose Name.
